Planning for a Decline in Your Health – Avoiding Crisis Situations

Start the year planning for Healthy Aging In this program we’ll kick start the conversation about what happens when you have a serious health situation that lands you in the hospital. We’ll focus on what happens when you cannot speak for yourself. Then we’ll look at what  comes after hospitalization. The hospital says you’re ready  for discharge and you need to know what resources are available to help take the next steps.
