As the holidays and colder weather arrive, it is time to get vaccinated!  

Costco Pharmacy will be at The Center giving vaccines on November 13 from 1:00-4:30 pm and November 16 from 2:00-6:30 pm.  

If you want a vaccine, you need to pick up a Costco Pharmacy Immunization Consent form at the Welcome Desk. Complete the form and check the vaccines you would like to receive. If you would like the RSV vaccine, check the "other" box and write in RSV. 

Vaccination Consent forms are due by Friday, November 10th  at noon! Be sure to turn in your form to the box on the date you want the vaccine (Monday or Thursday). Costco will prepare the vaccines  and bring them to The Center. You can get your vaccine anytime during the time slot for your date.

If you turn in your form and find that you will be unable to get your vaccine, please let us know. Contact susan@thecentercville.org.
